Please contact us at support@mypocdoc.com and we will get a replacement out to you as quickly as possible.
What information do we need?
To help us process your free replacement without delay, please include the following in your email:
- Your email address — the one used to create your PocDoc account.
- If you haven't created an account yet, please use the email address you used to place your order with us
- A photo of the inside of the lid of your PocDoc box — the sticker with the manufacturer's information on it
- Your activation code — found on the back of the blue box above the barcode
- Where you got your kit from — for example, a pharmacy, online, or supplied by a healthcare provider
- Details of what is missing or broken — so we can make sure your replacement is complete
As soon as we have all the information we need, we will send your replacement without delay.
